Creating an account is the first step toward joining Known Practice.

Next, complete your provider profile.

Please set aside at least 10 minutes to complete the full profile in one sitting. We can review and publish it only after all required sections are complete. Creating an account alone does not submit an application or create a directory listing.

After you create your account, we’ll email you a 6-digit verification code. Check Spam, Junk, or Promotions if it does not appear in your inbox.
